VIC - Powers of Attorney forms
In accordance with the Powers of Attorney Amendment Regulations 2021, the following forms have been updated:
- Enduring Power of Attorney (Form 1) [LL-VIC-WPA-046]
- Revocation by Principal of Enduring Power of Attorney of appointment of Attorney or Alternative Attorney (Form 2) [LL-VIC-WPA-049]
- Appointment of supportive attorney (Form 5) [LL-VIC-WPA-053]
- Revocation by Principal of Support Attorney or appointment of Attorney or Alternative Attorney (Form 6) [LL-VIC-WPA-054]
