LEAP has updated the following Children's Court of Western Australia suite of forms in accordance with official updates by the Court:

1. Application (Entry and Inspection) [LL-WA-CHC-003]
2. Application (Protection and care of children) - Form PC002 [LL-WA-CHC-004] has been replaced with the Application initiating care proceedings [LL-WA-CHC-004]
3. Application for early listing before the president [LL-WA-CHC-006]
4. Request to inspect or obtain a copy of a Court record [LL-WA-CHC-017] has been replaced by the Request to inspect or obtain a copy of a Court record (civil) [LL-WA-CHC-017]
5. Witness summons to give oral evidence [LL-WA-CHC-024] has been renamed Witness summons to give oral evidence (Criminal)
6. Witness summons to produce a record or thing [LL-WA-CHC-026] has been renamed Witness summons to produce a record or thing (Criminal)
7. Application to cancel responsible person or surety undertaking [LL-WA-CHC-010] has been replaced by the Application to cancel bail undertaking by responsible person and issue of warrant or notice to attend [LL-WA-CHC-010]
8. Notice of ceasing to act [LL-WA-CHC-016] has been replaced by the Notice of acting or ceasing to act [LL-WA-CHC-016]
9. Video conferencing CCTV booking and audio link request form [LL-WA-CHC-019] has been replaced by the Electronic attendance request form [LL-WA-CHC-019]

LEAP have also removed the Notice of address for service [LL-WA-CHC-015] as we have been advised by the Court that this form has been amalgamated with the Notice of ceasing to act to form the Notice of acting or ceasing to act.
