Updated FCFCOA Form
LEAP has updated the following forms in the Federal Family law matter type and in accordance with the official Federal Circuit and Family Court of Australia update:
- Affidavit [LL-FED-FCFCOA-075]
- Affidavit of proof of signature [LL-FED-FCFCOA-080]
- Affidavit of service [LL-FED-FCFCOA-081]
The Affidavit [LL-FED-FCFCOA-075] has been updated in accordance with the official Federal Circuit and Family Court of Australia November update to include title and organisation, along with family name for Applicants and Respondents.

Instructional Information
Some of the forms that we are updating may contain instructional pages that can be removed before printing. Where this is the case we will place these pages in hidden text. This means that upon printing your document the instructional pages will not print.
To turn hidden text view on or off on a document you must do the following:
- Open a Word document
- Select File
- Select Options
- Select Display
- Tick the checkbox next to Hidden text to turn hidden text view on, or untick it to turn hidden text view off
- Ensure that Print hidden text is unticked
- Select OK
