SA - Uniform Civil Rules (UCR) amendments - Prescribed forms

In accordance to recent modifications to prescribed forms contained in Schedule 7 to the Uniform Civil Rules 2010, the following forms have been updated:
  • Affidavit (Form 12) [LL-SA-UCR-004]
  • Affidavit - in support of application for admission (Form 12A) [LL-SA-UCR-005]
  • Affidavit of personal injury particulars (Form 10) [LL-SA-UCR-006]
  • Affidavit of proof of service (Form 42) [LL-SA-UCR-050]
  • Certificate of readiness for trial [LL-SA-UCR-075]
  • Exhibit front sheet to Affidavit or Statutory Declaration (Form 14) [LL-SA-UCR-012]
  • Statutory declaration (Form 13) [LL-SA-UCR-045]
  • Statutory declaration - in support of registration (Form 13A) [LL-SA-UCR-046]
  • Written submissions (Form 190) [LL-SA-UCR-137]
In addition to the updated forms, the following new forms have also been added:
  • Summary of argument (Form 89) [LL-SA-UCR-166]
  • Written submissions (Form 90) [LL-SA-UCR-167]
  • Notice to party - minor civil review (Form 191) [LL-SA-UCR-168]
  • Written case on minor civil review (Form 192) [LL-SA-UCR-169]
REMINDER: from Monday 21 September 2020, CourtSA is being mandated for legal practitioners in the civil jurisdiction. Court SA advises this means: Anything that can be lodged using CourtSA must be. For instructions on how to lodge prescribed forms using CourtSA click here. Anything that cannot be lodged using CourtSA because:
    • it is a lodgement that cannot be done using CourtSA;
    • there is data migration or technical issue associated with the case; or
    • access to the case has been requested but not yet granted
can be lodged via email or in person at a CourtSA Registry Services counter.
