VIC - Interactive PDF forms - Enduring Power of Attorney Appointment
The following forms are now available in LEAP in interactive PDF format in the Powers of Attorney forms folder:
- Enduring Power of Attorney Appointment – interactive short form [LL-VIC-WPA-066]
- Enduring Power of Attorney Appointment – interactive long form [LL-VIC-WPA-067]
The interactive short form [LL-VIC-WPA-066] allows for the appointment for one attorney and up to two alternative attorneys, whereas the interactive long form [LL-VIC-WPA-067] allows for the appointment for up to four attorney and up to three alternative attorneys for each attorney. Sections 6-8 and the Continuation sections of the form are currently not interactive – please complete these sections manually as required.
The LEAP toolbar within the Interactive PDF (generally on the side panel,More Tools) provides the option toSave & Close thePDF as well as toUpdatethe PDF with any new data entered.
Clicking onUpdatewill also re-run any questions should you need to change the responses to any questions - these forms contains prompts for the selection of principal, attorney and alternative attorneys (where applicable).
