VIC - Powers of Attorney

Enduring powers of attorney are changing on 1 September 2015, with the commencement of the Powers of Attorney Act 2014. Note: The Powers of Attorney Act does not affect medical powers of attorney – the enduring power of attorney (medical treatment). The enduring power of attorney (medical treatment) will continue to be regulated by the Medical Treatment Act 1988. The following new forms have been added to LEAP:
  • Enduring Power of Attorney – Form 1 (LL-VIC-WPA-026)
  • Revocation by Principal of Enduring Power of Attorney or Appointment of Attorney or Alternative Attorney – Form 2 (LL-VIC-WPA-027)
  • Notification by Attorney – Form 3 (LL-VIC-WPA-028)
  • Resignation by Attorney or Alternative Attorney – Form 4 (LL-VIC-WPA-029)
  • Appointment of Supportive Attorney – Form 5 (LL-VIC-WPA-030)
  • Revocation by Principal of Supportive Attorney Appointment or Appointment of Supportive Attorney or Alternative Supportive Attorney – Form 6 (LL-VIC-WPA-031)
  • Resignation by Supportive Attorney or Alternative Supportive Attorney – Form 7 (LL-VIC-WPA-032)
The following forms have been deleted in LEAP:
  • Enduring Power of Attorney (financial) – Form 1 (LL-VIC-WPA-001)
  • Enduring Power of Guardianship (LL-VIC-WPA-003)
  • Revocation of Appointment of Enduring Guardian (LL-VIC-WPA-005)
  • Revocation of Enduring Power of Attorney (Financial) – Form 2 (LL-VIC-WPA-006)
  • Statement of Acceptance – Must accompany Enduring POA (Financial) (LL-VIC-WPA-007)
  • Enduring Power of Attorney (Financial) Resignation of Attorney (LL-VIC-WPA-009)
