FED - Federal Circuit Court - Bankruptcy Forms

A review has been completed of the Federal Circuit Court Bankruptcy forms. These forms will now display a heading in the format similar to the Federal Court as these forms are now contained in Federal Court Bankruptcy Rules 2005. The following forms have been updated with the new style:-
  • Form 2 - Application(LL-FED-FCC-026)
  • Form 2 - Application (modified for application to set aside Bankruptcy Notice and/or to extend time for compliance) (LL-FED-FCC-024)
  • Form 3 - Interim application(LL-FED-FCC-029)
  • Form 4 - Notice of Appearance (LL-FED-FCC-030)
  • Form 5 - Notice stating grounds of opposition to application, interim application or petition (LL-FED-FCC-031)
  • Form 6 - Creditors Petition (LL-FED-FCC-032)
  • Form 7 - Sequestration order (LL-FED-FCC-033)
  • Form 8 - Referral of debtor's petition (LL-FED-FCC-111)
  • Form 9 - Summons for examination (LL-FED-FCC-112)
  • Form 10 - Application for summons to examine relevant person or examinable person (LL-FED-FCC-113)
  • Form 11 - Notice to creditors of annulment application (LL-FED-FCC-114)
  • Form 12 - Notice to creditors of application for review of Registrar's decision to make sequestration order (LL-FED-FCC-115)
  • Form 13 - Notice to creditors (LL-FED-FCC-116)
  • Form 14 - Applicant creditor's petition for administration of deceased person's estate (LL-FED-FCC-119)
  • Form 15 - Administrator's petition (LL-FED-FCC-118)
  • Form 16 - Arrest warrant (LL-FED-FCC-119)
  • Form 17 - Apprehension warrant (LL-FED-FCC-120)
All other forms contained in this profile have been reviewed however the heading of these documents has not changed and they are consistent with the forms from the Federal Circuit Court website. Minor formatting issues were found with the following forms :-
  • Affidavit of service (LL-FED-FCC-016)
  • Affidavit - instructions for completion (LL-FED-FCC-013)
  • Notice of discontinuance (LL-FED-FCC-020)
  • Notice of intention to withdraw as lawyer (LL-FED-FCC-102)
  • Notice of withdrawal as lawyer (LL-FED-FCC-103)
  • Notice of request to inspect (LL-FED-FCC-125)
  • Subpoena (LL-FED-FCC-023)
Forms deleted as no pro forma was found on the Federal Circuit Court website :-
  • Affidavit of service of creditor's petition to be sworn by process server (LL-FED-FCC-088)
  • Affidavit of service of bankruptcy notice to be sworn by process server (LL-FED-FCC-087)
