FED - Family Law Forms

In accordance with Family Law Amendment Rules 2012 (No. 1) the following forms have been updated (as noted below) and are to be used as of 7 June 2012. Please note the court has not advised of any crossover period with these forms:
  • Acknowledgement - information from a family counsellor or family dispute resolution practitioner(LL-FED-FLC-501 and LL-FED-FLC-601) - Page 1first note, advice to applicant - numerous changes
  • Application for consent orders(LL-FED-FAM-024) - Pge 3question 11, page 10first line, pages 23 and 24updated question 8
  • Application for consent orders kit(LL-FED-FAM-025)
  • Annexure to draft parenting consent order (LL-FED-FAM-019) - Page 1 instructions, Part B certification
  • Application for divorce kit(LL-FED-FLC-510 and LL-FED-FLC-610) - Part E, notice of application for divorce
  • Divorce service kit (LL-FED-FLC-518 and LL-FED-FLC-618)
  • Initiating application (LL-FED-FLC-524 and LL-FED-FLC-624) - Form format has changed, Part F
  • Initiating application kit (LL-FED-FLC-525 and LL-FED-FLC-625)
  • Form 4 - Notice of child abuse or family violence (LL-FED-FLC-526 and LL-FED-FLC-626) - Form has been renamed, new note before Part A, question 3, Part E, Part G, Part H
  • Parenting questionnaire (LL-FED-FAM-077) - Part A, question 13, Part B question 14
  • Response to initiating application (LL-FED-FAM-085) - Form format has changed, Part E, Part H
  • Application for divorce (LL-FED-FLC-509 and LL-FED-FLC-609)
