I can see that you have raised this with our HelpDesk and that the issue seems to be caused by a third-party App being enabled in Outlook.
From the case notes, it looks like this error does not prevent your colleague from actually attaching the document. Is this still the case?

You are correct. We can work around the issue but my preference would be to resolve the issue.
The problem going away when disabling the addin is not the issue. The error is to do with SQLite which is only used by LEAP. I do feel like this is LEAP avoiding the issue and not finding a solution for a problem
